Application and Forms Deadlines

​​​​​​The Application deadline is February 17th, 2023 or when divisions fill. 

  • Use Early Bird code EBMM23 to save $25 until Feb. 4th, 2023.

All required forms due February 22nd, 2023 (Player Waivers and Proof of Age)


Team Manager Account

Manage the team roster online to add, drop, or edit player information for the event.

  • To access the account, use the Team ID number received after being accepted into the event. 
  • The maximum roster size is 12 players.
  • Manager must check the box indication concussion training has been completed. (This is a requirement of the state of Minnesota Statute 121A.37. The U.S. Center for Disease Control and Prevention has a free, online course entitled “HEADS UP” is accessible by the link here.)
  • Players must be added to the roster before parents can complete the online NSC waiver.

Log in to an existing NSC Event HQ Account

This is a youth event - All waivers must be completed by the parent or legal guardian of each player (even if they are 18 or older). A parent or legal guardian must set up an NSC Event HQ account. The youth player must then be added as a participant on the parent account.

File Uploads

No more emailing and faxing!

Team managers and/or parents can securely submit all required documentation (required “forms”) by using the new “File Upload” feature in the NSC Event HQ portal.   

Log into your NSC Event HQ account

(if you do not have an NSC Event HQ account – click HERE to find out how to create one) 

From the list of options found under the “Customer Tools” menu, select the one labeled “File Upload”.  After finding/searching for the event you wish to upload documents for, you will be asked to provide your Team ID. 

After providing your valid Team ID you will be able to upload files to the NSC registration system.  Only files with the following extensions are allowed for uploading:  “png”, “pdf”, “gif” and “img”. 

Proof of age

  • Proof of age is required for all players
  • Youth players are required to present proof of age at Final Team Check-in: Player pass, copy of birth certificate or passport.
  • Adult players must be able to produce proof of age, if challenged: Driver’s license, player pass, copy of birth certificate or passport.
  • Player pass from previous season may be used for youth or adult players.

Concussion Training

All coaches involved in a National Sports Center YOUTH event are required to take concussion training as dictated by Minnesota Statute 121A.37. The U.S. Center for Disease Control and Prevention has a free, online course entitled “HEADS UP” is accessible by the link here.
