Application and Forms Deadlines
- Rosters are due November 4, 2022
Required Forms
- All players must be rostered and have completed the online NSC Liability Waiver (see below) in order to be eligible to play.
- Roster freeze date: November 24, 2022
- Players not on the roster and/or players without a current liability waiver on file by dates indicated above are not eligible to participate in the league and will be removed from the roster by the league registrar. Ineligible players will result in a forfeit. See rules.
- Rosters are frozen prior to playoffs, meaning that no revisions may occur to your team roster.
- Revisions must be made 48 hours prior to game time.
- Official rosters will be available to referees by the league monitor.

- You can manage your team's roster online, making it easy to drop and add players using your team manager account.
- You will need your team ID number you received after registering.
- Please note that the team roster must be completed before players can complete the online NSC Liability waiver.
Liability Waiver
Waivers due by November 5
- This is an ADULT event
- All players must be 18 or older and must complete the Liability Waiver for themselves
Players are NOT to be listed under the managers or other players account they must have their own NSC Event HQ account

Proof of Age
- Players must be able to produce ID in the event of a challenge. Ineligible players will result in an automatic forfeit. See rules.
- Proof of age includes any form of Minnesota State ID, driver’s license, passport, birth certificate, and/or player pass.
