Application Deadlines

Applications

  • Session I deadline: October 28, 2022         
  • Session II deadline: December 21, 2022   
  • Session III deadline: February 17, 2023    

Applications are accepted on a first come, first served basis until deadline or until divisions fill.

Required Forms and Deadlines

  • Session I: Due Wednesday, November 2, 2022
  • Session II: Due Wednesday, December 28, 2022
  • Session III: Due Wednesday, February 22, 2023
  • All players must be identified on the current team roster and have completed the online NSC Liability Waiver.
  • All Player must submit POA (Proof of Age)  
  • Full payment due March 1
  • Rosters are frozen on the following dates. No roster adjustments will be allowed after this date.
    • Session I: Friday, November 25, 2022
    • Session II: Friday, January 20, 2023
    • Session III: Friday, March 17, 2023
  • Ineligible players will result in a forfeit.
  • Current official team rosters are available to the referees at the monitor’s desk in the event of a challenge. See rules.

  • You can manage your team's roster online, making it easy to drop and add players using your team manager account.
  • You will need your team ID number you received after registering.
  • Please note that the team roster must be completed before players can complete the online NSC Liability waiver. 

Liability Waiver

This is classified as a youth event . The NSC Event HQ account must be set up with the parent or legal guardian as the account holder and the player added as the participant.

Once the roster has been entered direct parents to complete the waiver using the process outlined below in the NSC Event HQ. (Only need one account per family.)

Proof of Age

  • Proof of age must be received  by the required forms deadline
  • Proof of age is required for all youth players - See below for file upload instructions 
  • POA will not be accepted at the game.
  • Proof of age includes any form of Minnesota State ID, driver’s license, passport, school ID with birth date, birth certificate, and/or player pass.
  • Please see the age categories for the league.

Team managers and/or parents can securely submit all required documentation (required “forms”) by using the “File Upload” feature in the NSC Event HQ portal.   

Log into your NSC Event HQ account

(if you do not have an NSC Event HQ account – click HERE to find out how to create one) 

From the list of options found under the “Customer Tools” menu, select the one labeled “File Upload”.  After finding/searching for the event you wish to upload documents for, you will be asked to provide your Team ID. 

After providing your valid Team ID you will be able to upload files to the NSC registration system. 

Only files with the following extensions are allowed for uploading:  “png”, “pdf”, “gif” and “img”. the cap file size is 2MB.

The Filename should be 20 characters or less.

Concussion Training

All coaches involved in a National Sports Center YOUTH event are required to take concussion training as dictated by Minnesota Statute 121A.37. The U.S. Center for Disease Control and Prevention has a free, online course entitled “HEADS UP” is accessible by the link here.
