Application and Forms Deadlines

Applications due October 6, 2023 or when brackets fill

  • All required forms are due on Friday October 13th, 2023

Required Forms

 

  • Online NSC Roster  with jersey number for each player (no duplicate numbers) 
  • Online NSC Waiver completed for each player
  • POA - proof of age submitted for each Youth player 
  • Remaining balance paid

Team Manager Account

Kick-or-Treat Team Manager Account

Manage the team roster online to add, drop, or edit player information for the event.

  • To access the account, use the Team ID number received after being accepted into the event. 
  • The maximum roster size is 12 players.
  • Manager must check the box indication concussion training has been completed. (This is a requirement of the state of Minnesota Statute 121A.37. The U.S. Center for Disease Control and Prevention has a free, online course entitled “HEADS UP” is accessible by the link here.)
  • Players must be added to the roster before parents can complete the online NSC waiver.
  • The player’s date of birth (DOB) is entered by the parent on the NSC Liability Waiver
  • A player’s DOB is electronically monitored for age eligibility. If a player’s DOB continues not to be numerically displayed, the player may be too old for the age bracket. Ask the parent to review the DOB for accuracy. All players must be age-eligible for the assigned age bracket; DOB on the roster must match DOB on proof of age.
  • To monitor waiver progress  ‘View Roster’ from the Roster Menu. An ‘X’ displayed in the WV (waiver) column indicates a completed waiver.

Liability Waiver

All required forms are due by noon, the day prior to game day.

Log in to an existing NSC Event HQ Account

This is a youth event - All waivers must be completed by the parent or legal guardian of each player (even if they are 18 or older). A parent or legal guardian must set up an NSC Event HQ account. The youth player must then be added as a participant on the parent account.

Team Check-in

Team Manager or Representative will need to check-in the team 30 min. prior to their first game. 

 All required forms are due by Friday October 13th. 

  • Proof of age  (POA) is required for all youth teams/players can use a Player pass, copy of a birth certificate, or passport. Email to Registrar.

  • Adult players must be able to produce proof of age if challenged: Driver’s license, player pass, copy of a birth certificate, or passport.

Concussion Training

All coaches involved in a National Sports Center YOUTH event are required to take concussion training as dictated by Minnesota Statute 121A.37. The U.S. Center for Disease Control and Prevention has a free, online course entitled “HEADS UP” is accessible by the link here.
